Welcome New Patients!

Dear Patient, we would like to take this opportunity to welcome you to our practice.

On your initial visit to our office we will review your medical and dental history. Please make sure you have a list of any medications you take. If you need pre medication for your dental visit please let us know.

Patient Information
Smile and TMD evaluation

Dental x rays are essential for your examination. If you have had a full set of dental x-rays (14-16 films) within the last 3 years, please obtain a copy and bring them to your appointment. If we do not have these x rays at the time of your appointment, we will take a new set.

Usually you will first see one of our hygienists followed by the dentist. Your blood pressure will be taken, any x rays required will be taken, you will have your teeth cleaned, oral hygiene instructions will be presented and the doctor will do a full intra oral examination. This will include an oral cancer screening, a TMD evaluation, a periodontal evaluation and an examination of all your teeth and soft tissue. All of our findings will be explained to you in detail. If treatment is necessary we will provide you with a treatment plan and discuss payment options.

If you have dental insurance please bring your identification card or handbook with you. If there are any questions about your coverage we would appreciate you finding out about this in advance of your appointment.

Deductibles and co-payments are due at the time of treatment. We accept cash, personal checks, Care Credit, Amex, Visa, and MasterCard for your convenience. We will do our best to estimate your co-payments. You are responsible for all expenses that are not covered by your insurance.

The office generally sends billing statements at the end of each month. All open accounts receive a statement. If there is an * by a service this means there is still an outstanding insurance claim. If you receive a statement that indicates all insurance has been paid, payment is expected in full.

All appointments are confirmed by telephone one to two days in advance. Please provide us with the best telephone number to reach you. If you need to rearrange your appointment for any reason we would appreciate notification at least 24 hours in advance during our business hours. It is very important to be prompt for your appointment. We have reserved this time especially for you. We will always do our best to be on time.

Office hours run Monday to Thursday with alternating Fridays and Saturdays .Our office is generally closed for lunch on Monday, Wednesday and Thursday between 1:00PM and 2:00PM. If you call and reach our voice mail please leave your name and telephone number and we will get back to you as soon as possible. Our voice mail is also checked regularly during non business hours.

Recare visits are vital to maintaining your dental health. Recare intervals vary depending on your personal needs. We strongly urge you to schedule your next recare appointment in advance. We will provide you with an appointment card that has the date and time of your appointment and our telephone number on it. Approximately two weeks prior to this appointment you will receive a card in the mail reconfirming this date. If there is a problem with the time or the day we would appreciate a phone call as far in advance as possible so that we can provide for you another convenient appointment.

Payment Options

Our office participates with MetLife, Guardian, Horizon Blue Cross Blue Shield of NJ, Cigna and Delta Dental of NJ. We will be happy to submit claims to all other carriers.
